The Fetal Ejection Reflex: When Your Body Pushes Without You
What if you didn’t have to be told when to push?
For some people during an unmedicated birth, there comes a moment when pushing doesn’t feel like a decision at all. The body simply takes over.
A powerful, involuntary urge can cause the abdominal and uterine muscles to contract in a way that feels almost impossible to resist.
This phenomenon is commonly called the fetal ejection reflex (FER).
And if you’ve never heard of it before, you’re definitely not alone.
What Is the Fetal Ejection Reflex?
The fetal ejection reflex describes a series of powerful, involuntary contractions that can occur as a baby descends through the birth canal.
Instead of consciously thinking, Okay, now I need to push, the laboring person may feel their body begin bearing down automatically.
Think of other reflexes your body performs without asking permission.
You don't consciously make yourself sneeze. You don't decide to cough when something irritates your airway.
Similarly, during an involuntary bearing-down reflex, the muscles involved in birth may suddenly seem to take over.
What Does It Feel Like?
People experience pushing very differently, so there isn't one universal sensation.
For those who experience a strong involuntary urge to push, it may feel like:

An overwhelming downward pressure

A sudden and uncontrollable urge to bear down

Deep grunting or vocalizing that happens instinctively

The abdominal muscles tightening without consciously engaging them

A feeling that the body is pushing on its own

A dramatic change in the intensity or character of contractions
Some describe being surprised by just how little conscious control they seem to have over it.
The body isn't politely suggesting that you push.
It's doing it.
What Causes It?
As baby moves lower, pressure and stretching within the pelvis, vagina, and cervix can trigger neurohormonal responses involved in labor.
One important part of this process is the Ferguson reflex.
Pressure from the descending baby stimulates nerves, which signal the brain and contribute to the release of oxytocin. Oxytocin strengthens uterine contractions, which can move baby farther downward—creating additional pressure and continuing the cycle.
In some births, this process can culminate in an extremely strong involuntary bearing-down response.
Birth physiology is fascinating because so much of it happens without conscious direction.
Is the Fetal Ejection Reflex the Same as Pushing?
Not exactly.
During coached or directed pushing, a laboring person may be instructed when to push, how long to push, when to hold their breath, or when to stop.
Spontaneous pushing is different. Instead of following a count or command, the person responds to what their body is naturally telling them to do.
And an intense involuntary bearing-down reflex takes that idea even further—the urge can become so powerful that not pushing may feel nearly impossible.
Different births and medical circumstances call for different approaches, and there are situations in which a healthcare provider may recommend specific pushing techniques.
Does Everyone Experience It?
No.
And that's important.
You do not need to experience the fetal ejection reflex to have a normal, physiological, empowering, or successful birth.
Some people experience unmistakable involuntary pushing.
Others experience a gradually increasing urge to push.
Some need active coaching.
Epidural anesthesia and other circumstances may also change how pressure and the urge to push are perceived.
There is an enormous range of normal birth experiences.
What Does This Have to Do With Waterbirth?
One reason some laboring people enjoy water during labor is the freedom it can provide.
Warm water and buoyancy can make it easier to change positions and respond instinctively to what the body is doing. A person might kneel, squat, lean forward, float, or move between positions as labor progresses.
During the pushing stage, having the ability to follow those physical instincts can feel especially valuable.
Water doesn't guarantee that someone will experience an involuntary pushing reflex—and it doesn't need to.
It simply provides another environment in which a laboring person may be able to move and respond to their body.
Your Body May Know Before Your Brain Does
One of the most remarkable things about birth is how much happens beneath conscious control.
Hormones shift.
The uterus contracts.
Baby descends.
Pressure increases.
And sometimes the body responds with an unmistakable message:
It's time.
Whether pushing arrives as a gradual urge or an overwhelming reflex, learning about the physiology of birth before labor can make those sensations feel a little less mysterious when the big day arrives.
Because sometimes, your body really does know what to do before you have time to think about doing it.
